Do I have to fill in every field?

No. Only the fields you fill in are included in the card — any single field is enough to generate a code. Leaving fields empty also keeps the QR code smaller and easier to scan.

A QR code has limited capacity, so longer content needs more modules. Keep the card concise, or lower the error-correction level if scanning becomes unreliable at small print sizes — higher levels add redundancy but increase density.

No. The contact details are stored inside the QR code itself, not behind a link, so a printed code never changes. If your details change, generate and print a new code.
